Where it breaks
Records are self-reported, so a relocation can look identical to a new opening. CMS itself warns: "Issuance of an NPI does not ensure or validate that the Health Care Provider is Licensed or Credentialed."

Before you type your NPI
- I would have heard about it.
- A registration and a grand opening are different dates. Had you heard?
- These will be false alarms.
- Some will. A relocation can look like an opening, so every alert carries the raw record for you to judge.
- Do you cover veterinary?
- No. Vets do not register NPIs; only eight veterinary organizations hold one in all of California.
